Consultation Forms and Day of Visit Information
Welcome to STARS Plastic Surgery and we are excited to be here for your journey to enhancement or restoration.
We are located at 8042 Wurzbach Road, San Antonio TX 78229 on the first floor of Physician Plaza II, next to the Methodist Specialty and Transplant Hospital. Ample parking is available on-site but be sure to bring your parking ticket so we can validate it for you. If you are coming in for a new cosmetic consultation, please note that there is a $75.00 consultation fee which will be credited back to you for any services you purchase.
To help decrease the waiting time during your visit, please fill out our new patient paperwork. This will help us get to know you better and be prepared for our meeting! Follow the link below to complete the intake form. At the conclusion of filling out your intake form, you will automatically be directed to fill out your treatment consent forms. Finally, you will be directed to a site where you can create an account to our patient portal where you can order products, receive your estimates and even accumulate points for rewards!

To protect our patients, family members and staff, all STARS Plastic Surgery patients and visitors will need to complete a day-of-visit questionnaire regarding their risks of disease transmission. This questionnaire is intended for everyone who visits our practice location(s) IN PERSON and needs to completed for each 24 hour period. It applies if you are either the patient or if you are accompanying a patient. We thank you for taking the extra time to help us maintain a COVID free healthcare workplace.
